“PATHWAYS TO ELIMINATING COLORECTAL
CANCER”
Goal: To increase colon cancer prevention and
early detection practices.
Objectives:
1. Sponsor an educational event at a local venue
to inform attendees of proactive
measures for reducing and eliminating CRC. Format
includes:
A. Keynote Presentation
Mark Clanton, M. D., Chief
Medical Staff Officer, American Cancer Society, High Plains Division
B. Panel Presentation

Clinical Practice Physician(Internal Med./Family
Practice)

Ancillary health professional (RN, Pub. Health,
Patient Navigator)

Hope Through Grace screening program participant

2. Promote increased screening rates for
uninsured, underinsured.
3. Promote increase participation rates of
insured who at risk and have not had colonoscopy screening.
Target Audience:
Cancer advocates, survivors, caregivers, health care
providers, and health educators will find latest data, research and practical
information that will increase their knowledge and understanding of evidenced
based programs and practices for eliminating colorectal cancer.
Intended Outcomes:
A
community-wide education event that is designed to:
1. Inform
participants of colorectal cancer prevention, incidence, screening practices
and community resources.
2. Improve
office practice of clinicians.
3. Provide
evidence-based tools and strategies.
4. Increase
screening rates for colorectal cancer.
